I don’t think documentation is anyone’s idea of a good time, but it is an important part of the psychotherapy process. Accurate documentation is required for insurance reimbursement and regulatory standards. It also can help the clinician process the session and what just happened. I don’t always love writing my case notes, but I am always grateful when I am able to go back to them. Here’s my breakdown of a common note format, DAP (Data, Assessment, Plan).
